We typically use two different types of paper for use in dive surveys. 

1- for times when we only need a few forms we use Copy Tuff 4mil White
Opaque Film, which can be used in a photocopier. Approx 60$ for 100
sheets. 

2- for surveys where we need several hundred copies of the same form, we
use Yupo (57lbs weight) paper ( http://www.yupo.com/ ). As far as I know
the Yupo paper cannot be used in a photocopier so we have a printing
business print the forms for us. This option works out cheaper if
printing over 500 sheets.
